HOME CARE

Do you dream of staying in your own home as long as possible but need some help to make that dream a reality? If you want to stay in your own home as you get older, but need some help with things like cleaning and preparing meals, or with transport so you can go shopping or attend appointments, a Home Care Package may help you.

  • Are you an older person who needs some help to stay in your own home?
  • Are you struggling with some aspects of living at home, but you’re still able to manage most of the time?
  • Do you think if you don’t get some help at home, you might have to go into an aged care home before you’re ready?

If you take up a Home Care Package you will pay:

  • A basic fee of up to 17.5% of the single basic Age Pension
  • An income-tested care fee if your income is over a certain amount.

Clients in receipt of a package have a St Carthage’s representative who works collaboratively with you and your advocate to identify your individual goals. Once your goals are identified, we will support you to choose what services and goods are required to achieve your goals. Clients are encouraged to consider a wellness and re-ablement approach when planning their package. Historically services provided included:

  • Personal care services – assistance with showering or bathing, dressing
  • Domestic services – assistance with washing and ironing, house cleaning, gardening, basic home maintenance, home modifications related to your care goals/needs
  • Transport to assist you to shop, visit your doctor or attend social activities
  • Clinical care – nursing and other health support including physiotherapy (exercise, mobility, strength and balance), services of a dietitian (nutrition assessment, food and nutrition advice, dietary changes) and hearing and vision services.
  • Continence Aids – pads, toileting aids.

Following 1 July 2015, with the implementation of your individual budget, home care packages will be delivered on a CDC basis. Your St Carthages representative can assist you to administer your budget, ensuring you have choice and control to choose your goods and services as well as who will deliver those services.

Through your Home Care package, St Carthage’s can assist you to have the opportunities to alleviate loneliness, helplessness and boredom and do the things that are meaningful to you.

All clients must be assessed by the Aged Care Assessment Team who will determine what level of package you require to support you to stay in your home. A member of our team will then arrange to meet with you to discuss your needs and goals and determined how we can assist you to meet them.

PERSONAL CARE

Personal care services are available to support you to stay in your own home. This includes everyday tasks such as bathing and getting dressed.

A broad range of services can be provided to meet your needs. These may include:

  • assistance with eating
  • bathing
  • going to the toilet
  • grooming
  • dressing getting in and out of bed
  • moving about the house

In most cases, you will receive these services in your own home.

If you only need a couple of services to help you stay living in your own home, it is best to access them through the HACC Program. This program can assist you if you need extra help to stay in your own home longer. If you have more complex needs, a Home Care Package may be right for you. You can access similar services to the HACC Program, but on top of this, the services are coordinated and tailored to meet your specific needs.

The Australian Government pays for the bulk of aged care in Australia. But as with all aged care services, it’s expected you’ll contribute to the cost of your care if you can afford to do so. You will never be denied the care you need because you can’t afford it. Any fees will be discussed and agreed upon with your support planner before you receive the relevant services.

DOMESTIC ASSISTANCE

Domestic help services can support you with practical tasks such as general housework and shopping. It’s services like these that may help you to remain independent in your own home.

  • clothes washing
  • ironing
  • shopping

In most cases you will receive these services at home, while some tasks, such as shopping may happen when you are out and about.

SOCIAL SUPPORT

We know that one of the three plagues suffered by people is loneliness. If you would like to have a visit, go out with someone to do something in the community, or just want to have a cup of tea with a friendly face, we can tailor make a plan for you.

Individuals and their carers living in the community. You must be 65 years of age or over or 50+ years if you are Aboriginal and/or Torres Strait Islander.

The Social Support program covers the whole of the Northern Rivers from the Tweed down through Byronshire to Grafton and west to Kyogle.

Individuals, their carers or other community agencies can refer for this service.

The cost is minimal dependant on the location and in most cases the support offered will have no charge connected to the service dependant on locationand the support requested.

Social support aims to assist and support individuals and their carers to participate in community life, gain companionship from others in the community including the social activities, hobbies and interests, home visits, assisting with shopping, library visits, and other related activities.

Clients are offered the opportunity to socialize and participate in recreational, leisure activities out and about in their community. To assist individuals to remain independent living in their own homes with companionship on a one-on-one basis with community volunteers.

Companionship is the antidote to loneliness, by bringing variety and spontaneity into people’s lives and supporting the unpredictable interactions and happenings to take place which is the antidote to boredom and the promotion of self-esteem and healthy active ageing in our community.

Referrals can be received by phone or CIARR. After receiving the referral the client is contacted and an appointment is organized for an home visit to discuss the support, availability and explain the service to the client and family.

Individuals living alone, isolated from community activities.

RESPITE

Everyone needs to be able to attend activities, work, and have a rest. We offer respite services tailored to suit your needs whilst your family member receives outstanding care from our experienced community care workers. The Respite Team believe in the importance of well-being, happiness, joy and supporting clients in creating a life worth living.

The Senior’s Respite Service is funded by the Australian Government under the National Respite for Carer’s Program. This service will provide flexible respite to carers of frail older people, people with dementia and people with dementia and challenging behaviours. Respite can be provided in a number of ways and is tailored to meet the needs of the carer and the person they care for.

Available for people in the Lismore and Richmond Valley LGAs.

Anyone in the community can make a referral to this service by either telephone, fax or in person by contacting our office. Referrals can also be made by other service providers using a CIARR.

Respite can be provided:

  • In the persons own home on weekdays, evenings or weekends or Overnight if appropriate
  • Social days at the Senior Day Club based at Our Lady of Lourdes, 135 Dibbs Street, East Lismore on Tuesdays and Thursdays from 9.30am–2.30pm
  • Small group outings or individual outings to access activities in Lismore, Evans Head, Kyogle and Casino communities
  • Short Term Overnight Respite in our Respite Unit

In Home Respite $6 per hour
Centre Based (Day Club) $18 per day (meal included)
Cost may differ for outings and special events
Community Access $6 per hour (does not include meal)
Overnight In Home or ‘Jubilee Unit’ $52 / night until 10am
(No one is refused service due to an inability to pay. This will be discussed at the assessment).

Aims to improve the quality of life for both the carer and the care recipient and should endeavour to address goals such as:

  • Remaining active in the community for as long as possible
  • Reduce social isolation and promote health, well-being and independence
  • Provide opportunities for the carer to meet social, emotional, family and work needs
  • Provide opportunities for the recipient for social engagement, companionship and stimulation

The Respite Team have embraced the Eden Alternative to help improve the well-being of our care recipients and bring a little joy to their lives.

  • We create opportunities for recipients to give care as well receive it. This includes supporting school children and caring for plants and our mascot, Daisy the Cocker Spaniel.
  • We are providing opportunities for social interaction with others
  • We are encouraging recipients to become active in their community
  • We encourage recipients to be involved in their care plan as much as possible as well as planning day club events and activities.

Initial Assessment – When a telephone referral is received, a member of the Respite Team who answers the call will complete the Intake/Assessment Form to determine eligibility of the client and the services that best suit the needs of the carer and the recipient. A member of the Respite Team will contact the Carer to complete the assessment process. Once the Intake/Assessment process is completed, the respite care plan will be developed for the client.

  • Review Process – A review is completed every six months (at a minimum) with both the carer and the recipient. Not only do we like to know how the service is working for you, it’s a chance to catch up and see how you are traveling and whether we can support you with anything else. It’s also important to keep our information current, such as health and medication records of the recipient. All this helps us to provide the best service possible.

Access to this service is not automatic and carers are assessed on a case by case basis. Some factors our team will consider when assessing recipients are:

  • Are other services providing support?
  • Is the recipient high care of low care?
  • Is the carer experiencing significant stress?
  • Does the carer have other commitments such as work or other dependents?

DAY CLUB

Our Senior Day CLub offers a wide variety of activities which are carefully designed to benefit the health of every member. Daily programs are created to promote, strengthen, and stimulate skills of the physical, emotional, intellectual, and social domains. Members spend the day here with friends, while giving caregivers freedom and peace of mind.

Individuals and their carers living in the community. You must be 65 years of age or over or 50+ years if you are Aboriginal and/or Torres Strait Islander.

  • Senior Day Club operates from Our Lady of Lourdes, 135 Dibbs Street, East Lismore.
  • Monday and Wednesday 10am – 2pm
  • Friday’s for Italian Seniors 10am – 2pm. This day club is culturally specific for older Italians.

Referrals may be made by: Self, Family, Friends, Doctors, Community Service Providers or Aged Care Assessment Team.

$18 per day. This covers transport, morning tea and lunch. No one is refused service due to an inability to pay. This will be discussed at your assessment.

Provides Centre Based Day Respite for older people residing in their own homes. Clients are offered the opportunity to socialise, participate in recreational and leisure activities that are both therapeutic and enjoyable.

Clients are offered the opportunity to socialise and participate in recreational, leisure activities and our exercise program. To assist clients to remain independent living in their own homes.

he clients participate in deciding and organising what they wish to have included at the day club. Activities promote healthy active ageing, self-esteem and independence in a supported environment. To eliminate Loneliness and boredom.

Referrals can be received by phone or CIARR. After receiving the referral the client is contacted and an appointment is organized for an assessment, to discuss availability and explain the service to the client and family.

Priority is based on the client’s needs and availability.

St Carthage’s Community care provides a private service with a dedicated team that can offer you domestic assistance, personal care, and respite services. Our staff are professional, caring, and have a vast array of experiences that make them the best in the field. We know a little bit of help can make all of the difference.

Private service is available for those with short or long term needs -Individuals and organisations. The private service provides you with the assistance and support you need to achieve your goals of living independently in your own home.

Through the private service St Carthage’s can assist you to have the opportunities to do things that are meaningful to you combatting boredom.

All clients will have an initial assessment which includes a discussion of care needs and a home risk assessment.

There is no priority system in place for Private Services.

TRANSPORT

Is lack of transportation holding you back from joing our fantastic day club or other social activities? If so, give us a call. We provide a transportation service to attend these activities.

We also can provide transport in a wheelchair accessible vehicle for a variety of other services.
