St Carthage’s Community Care is a fully registered and approved local NDIS service provider that offers short and medium term accommodation and respite, direct support services, support coordination and plan management.  Our goal is to provide services that generate opportunities to enhance people’s sense of belonging and well-being.

Support Coordination

Support coordination helps participants to connect to NDIS and other supports, broker supports and services in line with a participant’s wishes and their plan budget, monitor plan budgets and support effectiveness and build capacity and capability to understand their plan, navigate the NDIS and make their own decisions.

Plan Management

Our plan manager can help you increase your financial and plan management skills, learn how to self-manage your plan, pay providers, increase your choice of providers, get NDIS plan budget reports and greater budget oversight.

We can take out the stress of managing the participants’ s plan with direct consultation with those involved.
